How to Schedule an Appointment

Informed consent is an important part of therapy, and that starts with the process of scheduling an appointment. Here's some information to help orient you to the process of becoming a new patient.

Step 1 | Reaching Out

Contact me via email, phone, Psychology Today, Therapy Den, or the form on my website. I'm happy to answer any questions you may have!

Step 2 | Cost of Appointments

If you’re planning on using your insurance benefits, read Step 2a. If you’re planning on paying out-of-pocket, skip to Step 2b.

Step 2a: Using insurance benefits. I accept CDPHP, Aetna, and MVP insurance plans. If you provide me with your

  • full (legal) name

  • insurance member ID #,

  • your date of birth

I’ll call your insurance company to check your benefits. I do this so there are as few surprises as possible when it comes to billing and utilizing insurance. Once I contact your insurance company, I’ll email you with what your benefits are. This will include whether you have a deductible and/or a copay and what the amounts of each are. If any information appears inaccurate or doesn’t sound 100% correct, I encourage you to call your insurance company to check your benefits and I’m also happy to call them again to confirm.

Step 2b: Private Pay. If you’re not planning to use insurance benefits to pay for appointments, the cost per session is $175.

Step 3 | Scheduling

I am currently only accepting new patients for telehealth (video) appointments only (we can move to in-person appointments in the future when COVID-19 numbers decrease if you’re interested and fully vaccinated with a booster). We can discuss potential appointment times via email or by phone. I allow for one reschedule of the first appointment, if necessary, but because my schedule is limited, I will offer referrals to other providers if additional rescheduling is requested.

Step 4 | Paperwork

My paperwork will be sent via SimplePractice, my HIPAA-compliant electronic health record. Initial paperwork includes the Psychotherapy Intake Form, Right to a Good Faith Estimate, and the Informed Consent. I’ll also send the Release of Information Consent if there’s anyone with whom you’d like me to be able to communicate (primary care, family members, psychiatrist, etc.). All paperwork must be filled out 24-hours prior to the first appointment.

Step 5 | Payment

Payment of any copays or session fees is required prior to your appointment.

Step 6 | Our First Appointment

I’ll send you a video link before our first appointment or I'll send you directions to my office via email.
